Sign before we roll up

I am the parent or legal guardian of the child(ren) listed and I give permission for them to take part in The Adventure Box programs, including sand play, sensory games, creative play, and movie nights, at my event in McKinney, Texas and the surrounding service area.

I understand play involves sand, small tools, paint, low furniture, a movie screen, and other children. There is a risk of scrapes, sand in eyes, stains, or similar ordinary play injuries. I accept that risk for my child(ren) and agree The Adventure Box, its owners, and staff are not liable for ordinary play-related injuries, lost items, or clothing stains, except in cases of gross negligence.

I will stay on site and remain responsible for my child(ren). I will tell staff about allergies, sensory needs, or anything that would change how we host them. Movie selections are the host family’s choice; The Adventure Box does not preview or rate the film.
